How we use your personal information for Shielded Patient / Self Isolation – After Care

The Council provides services for local communities and the people who live in them.  Undertaking this work means that we must collect and use information about the people we provide services to and keep a record of those services. Because we collect and use personal information about individuals we must make sure that they know what we intend to do with their information and who it may be shared with. 

We have summarised in this privacy notice some of the key ways in which we use your personal information for providing ongoing support to those names on the shielded patient list. This information should be read in conjunction with the Council’s corporate privacy notice.

1. Who we are, what we do

For individuals identified on the NHS’ Shielded Patient List and following Welsh Government advice that shielding has since paused, it means that these people are now able to go out in public, return to work and children can return to school.

It is recognised that many who were shielding, or who had been self-isolating, will be worried about returning to their day-to-day lives and about losing the support that has been available to them.  Welsh Government have advised that Local Authorities and other agencies must continue to work together and provide ongoing support/after care for people coming out of shielding to ensure they are confident in leaving their homes and are still able to access facilities.  

Therefore, Adult Services and the Community Development Team will make sure that residents who have needed support during shielding, or whilst self-isolating (if instructed to do so by the Councils Contact Tracing Team) will continue to receive support and help

Adult Services

Will provide Information, Advice and Assistance in accordance with the Social Services and Wellbeing Act (Wales) 2016; this will be done through a ‘what matters conversation’ to establish an individual needs and necessary action/onwards referrals made.

Community Services

Working in Partnership with RCT’s Community Support Group members, which includes Council staff and Community Resilience Volunteers, this small team respond to calls and e-forms from residents to provide the support they have asked for.

2. What and whose personal information we hold?

The Council will already hold information about you and your family as you were identified as being on the Shielded Patient List.  Of if you have contacted us requesting support

We will already have the following information about you and your family:

  • Name
  • Address
  • Date of Birth
  • NHS Identification number (if previously given)
  • Contact Details 
  • Health Information

3. Where does the service get my information from?

As a result of the pandemic, the NHS was asked to produce a ‘Shielded Patients List’. The list included the names of extremely vulnerable patients living in Rhondda Cynon Taf and was shared with the Council so that we could provide you with support during this difficult time.

You may have also told us more information about yourself whilst you were shielding and received support from us at that time.

If you have contacted the Council requesting support whilst you are isolating, we will gain information from you when you complete the online form or by ringing the helpline.

4. What will we do with your personal information?

We will use your information to provide you with:

  • Support to go shopping
  • Support to go out to collect a prescription 
  • Support to go out to walk a dog
  • Support to access a Community Group or Activity 
  • Friendly telephone call to maintain social contact
  • Support to find work, or received work related training 
  • Information and guidance about money or benefits
  • Information about the 'At Home' library service 
  • Any other support to come out of self-isolation or shielding such as Meals on Wheels, Lifeline

If you have agreed, we will also contact you to carry out a survey on how you have found the support you have received and what we can do to improve the service we deliver.

If you do not require any additional support, we will not contact you in future unless there is either a Local or National Lockdown.

5. What is the legal basis for the use of this information? 

Under the General Data Protection Regulation (GDPR), the Council also has a legal duty to process personal information which is necessary;

  • for the performance of a task carried out in the Public Interest (see Article 6(1)(e) and Article 9(2)(g)

6. Does the service share my personal information with any other organisation?

Yes, the Council will share your information with other Council services so that we provide you with the support that you have asked for. 

Depending on the support you need we may also share your information with our key partners/COVID-19 Steering Group. This includes but is not limited to;

  • RCT Core Steering group, such as but not limited to:
  • Council Services
  • Registered Social Landlords (such as Trivallis, Cynon Taf Housing, Rhondda Housing Association, Newydd Housing.)
  • Interlink (representing Community groups)
  • Foodbank
  • Age Connect Morgannwg
  • Cwm Taf Morgannwg Health Board
  • Citizens Advice Bureau
  • Wales Probation Service
  • Department for Work & Pensions
  • Homestart
  • The British Red Cross

We will also share the outcomes of any surveys we have carried out but all answers will be anonymous and your personal information will not be shared.

If there is a local or National Lockdown in future we may also share your information with Welsh Government 

7. How long will my information be kept?

We will only keep your personal data for as long as is necessary for the purpose for which we are processing it, unless we have a legitimate reason for keeping it, for example if you request assistance this information will be added to your social services record.

8. Your information, your rights

The General Data Protection Regulation (GDPR) gives you important rights including the right to access the personal information the services hold about you.
