An individual plan is a plan that is created using the outcomes agreed in a care plan.


This forms the basis of the plan and then together with your key worker, you will agree how you want to acheive your outcomes and the stages needed to meet them.

You have total input into this process, your key worker may make suggestions for you to consider but you have full input. 

When your individual plan has been finalised and agreed you will be provided with your own copy to keep.  A copy will also be placed in your file based in the ILS office and a copy given to the Care Coordinator.
