If you are an adult with a learning disability you may be eligible for our support to help you and your parents or carers.
Overview
If you are an adult with a learning disability and are eligible for our help, we will support you with a range of services to help you live as independently as possible and to support your parents or carers.
Who it is for (eligibility)
You will qualify for support from us if you have a professional diagnosis of a learning disability.
A diagnosis must include these 3 criteria:
- Significant impairment of intellectual functioning – a significantly reduced ability to understand new or complex information and to learn new skills.
- Significant impairment of adaptive functioning – a significantly reduced ability to cope independently.
- Be diagnosed before adulthood (before the age of 18) - resulting in a lifelong developmental impact.
Who it is not for
You will not qualify for support from us if you have:
- A learning difficulty such as ADHD, Dyspraxia or Dyslexia
- A Neurological condition such as Autism
- Have a had a brian injury in adulthood
These diagnosis alone will not qualify you for support through us. You can get further help and support for for these conditions through *** SPA or Care and Support (need to firm this up and add details)****
Your rights as a person with a learning disability
As an adult with a learning disability you have a legal right to have your voice heared and to have an active involvement in your life choices.
We can hold a best interest meeting to find out if you need help to make decision. We can work together with the you, and your family work out a plan.
If you feel you need further help, we can arrange an advocate who can help you to:
- get the information you need
- speak about what's important to you
- understand your rights

What we can do for you
If you have been referred to us we will:
- Assess your needs
- Make you a care plan
- Tell you how we can meet your needs
- Allocate you a dedicate support worker
You will have access to services such as:
- Day centre visits
- Volunteering or work experience
- Training to help you travel by bus or train in your local area
- Help to access supported living accommodation
- Support using direct payments which allows you to arrange your own care and support
- Help to arrange for your voice to be heared through an Advocate

Who to refer in
Individuals referred into our service must meet all of the learning disability eligibility criteria which includes:
- Cognitive assessment or equivalent clinical evidence of intellectual impairment
- Evidence of significant adaptive or social functioning difficulties
- Confirmation that impairments were present before the age of 18
