We know the way society is set up can sometimes be a barrier to your child doing what matters to them and can give your child less choice.
We will work with you to try to and overcome some of the things that get in the way. This will be detailed in the Care and Support Plan. The plan will be completed alongside you and will take into account all the strengths and resources that your family has. It will be regularly reviewed and will change as new needs arise, or no longer be required as outcomes are achieved.
