Enabling people to remain at home safely and independently. The Rehabilitation Service, can be therapy-led, or non-therapy led and supports Rhondda Cynon Taf residents aged 18 and over who are eligible for our assistance.
Our Intermediate Care and Rehabilitation Service may be able to help if you are:
- recovering from a recent illness or accident
- anxious about going home from hospital
- well enough to go home but worried about your ability to cope
- wanting to do more for yourself
We work to promote your independence by encouraging you to learn or re-learn basic living skills. This is a short-term service, so if you require ongoing support, we will work with you to explore suitable longer-term options.
The service provides support with day-to-day aspects of daily living by assisting, encouraging and supervising with tasks, such as:
- personal care
- meal preparation
- exercise programmes
- mobilising safety in the home and outdoors
- making sure you have the right equipment
- therapeutic activities
- referring onto other community services like the Safe@Home project and Telecare
We will assess your needs and discuss with you a plan of support. Together we will identify and agree your personal goals and targets. The goals will vary depending on your individual circumstances, and will be designed to help you remain as independent as possible in your own home.
You may find that, as part of your plan of support, specialist equipment will be useful to help you manage independently. If this is the case, you will be advised on how aids and adaptations can be provided to overcome difficulties to achieve independence.
Our Rehabilitation Service is provided free of charge for up to 6 weeks, but the service will only be given for as long as you remain in need of our assistance. If you have been identified as needing ongoing long term care and support, you will be charged in accordance with our Charging for Non Residential Social Services policy. We can give you more details at this time, so you won’t receive an unexpected bill.

Our Rehabilitation Service is registered and regualted as a Domiciliary Care Service with CIW.
