The Independent Living Service (ILS) is a service that works to support individuals to achieve the outcomes agreed in their care plans.
ILS provide support in a variety ways, in order to help people who have different needs, remain independent and build confidence to be a valued member of their community.
When we receive a referral, it is allocated to a member of the team who will then become the key worker for that person. An individual care plan is then agreed and created by the support worker, with the outcomes and wishes of the individual at the heart of the plan. Once created the care plan is signed by both the individual and the support worker and support with then begin.
